    • Summary email with provider options, wait times, and recommended next steps

    • Family member included in updates if desired

CHSP Reconnect & Reassess

For clients who: have CHSP but are not receiving services, or whose needs have significantly changed and may require escalation to a Home Care Package (HCP).

    • In-person consult (in-home or office, individual or couple)

    • Review of CHSP status, current health, function, and support gaps

    • Sistability team member (Jenn) added as My Aged Care Representative (if not already)

    • Call with My Aged Care to:

      • Request CHSP referrals or follow up on existing ones

      • Submit new service requests (if needed)

      • Request reassessment for HCP

    • Upload of supporting documentation to My Aged Care portal

    • Summary email with actions taken, outcomes, and next steps

    • Family email updates (if applicable)

    • Advice on private-pay options, if required

    • A formal reassessment request submitted — backed by a clear explanation of your changing needs

    • CHSP referrals activated or followed up so you’re not left waiting in limbo
