Available Reports:
1. Household Condition Report:
A household condition report is highly beneficial as part of a client care plan. Incorporating a household condition report into the care plan enhances the ability to tailor the care environment to the specific needs of the client, ensuring they can live safely and comfortably in their own home.
2. Exterior Condition Report:
Our exterior condition reports evaluate the safety and usability of the client’s outdoor areas, including gardens and access paths, which can significantly impact quality of life and client independence.
3. Changes in Client’s Wellbeing Report:
This report tracks any changes in the client’s physical, mental, and emotional health, providing crucial data for timely adjustments to their care plan and includes a minimum of 2 visits to the clients’ home to record and report findings over a period of time.
